Documents required from candidates for the 6-Year Medical Doctor Program: 

1. Application form log in to our internet registration platform

2. Original High School Diploma or certified copy (Secondary School Certificate), which must be translated into English or Polish, made by a sworn translator.
The Diploma must contain a clause certying the right to apply for admission to higher education institutions in the country where the High School Diploma was issued. 
    Additionally the Foreign High School Diploma must be authenticated:

• if Issued within the education system of an EU, EEA or OECD member states or the Hague Convention country the Diploma must be stamped with the Apostille seal 
• All other Foreign High School Diplomas must be legalised

3. Certificate of Proficiency in English (e.g. IELTS – minimum 6.5, equivalent score from TOEFL or Pearson Assessment English)
4. Bank transfer confirmation for the Recruitment fee of 85 PLN

 Documents required after admission:

1. CD with a photograph in electronic form (.JPG file, size 300 x 375 pixels, image resolution not lower than 300 dpi)
2. Two photographs (35 X 45 mm), taken against a light background, taken without headgear (as for passport)
3. Proof of health insurance valid in Poland (European Union citizens- EHIC card)
4. Copy of student visa
5. Health Certificate
